Vadim Petrochenkov 67d6ce4206 syntax_pos: NO_EXPANSION/SyntaxContext::empty() -> SyntaxContext::root() ...

For consistency with `ExpnId::root`.

Also introduce a helper `Span::with_root_ctxt` for creating spans with `SyntaxContext::root()` context
